Convert a Referral to Patient

Go to Referrals and click on an accepted referral. Review the demographic information already captured during the referral process.

Click Convert to Patient. WaiverCarePro creates a full patient record pre-populated with the information from the referral — name, date of birth, Medicaid ID, address, and MCO payer. You are taken directly to the new patient record.

On the patient record, complete any missing fields in the Demographics tab. Add emergency contacts on the Contacts tab. Upload the physician order and signed consent forms on the Documents tab.

Go to the Authorizations tab and create the service authorization using the information received from the MCO. Enter the procedure code, total authorized units, and the authorization period dates.

Go to Scheduling → New Schedule to assign a caregiver and book the first visit. The referral record is automatically linked to the patient and shows as Converted.
