Report an Incident
Document reportable incidents and submit to your state HCSIS system.
Create an incident record
Go to Compliance → Incidents. Click New Incident. Select the incident date, type (fall, medication error, behavioral, abuse/neglect, or other), and severity (minor, major, or critical).
Describe the incident
Enter a detailed description of what occurred, the immediate actions taken, and identify the caregiver and patient involved. Accurate documentation is critical for state reporting and legal protection.
Reporting requirements
Critical incidents must typically be reported to the MCO within 24 hours and to the state within 72 hours. WaiverCarePro tracks the incident date so you can see when reporting deadlines are approaching.
Submit to HCSIS
When ready to file with the state, open the incident record and complete the HCSIS fields: location, supervisor notified, MCO notified, law enforcement contacted (if applicable), and family notified. Click Submit to HCSIS and enter the confirmation number you receive.
Close the incident
Once the incident is resolved and all reporting is complete, click Mark Resolved. Enter the resolution date and a summary of the outcome. Resolved incidents remain in the audit trail but are excluded from the open incidents count.
